There is no application form for a Lab Affiliate Membership. If you are part of a lab whose Principal Investigator has a Lab Membership with the Biophysical Society of Canada, you will need to ask your PI to add you to their account as a “sub-account”, or ask them to share a sign-up link so that you can join their lab membership at no cost.
